Remote Wound Monitoring: What Providers Should Know

Remote wound monitoring is a workflow, not simply a folder of photographs. Providers need to know who collects information, how it is reviewed, and what happens when it raises a concern. Those decisions matter before choosing a device or AI feature.

01 · Define the service

Be precise about what is monitored.

HHS describes remote patient monitoring as using technology to collect health information outside a traditional clinical setting and share it with a provider. HHS: telehealth and remote patient monitoring. Wound-photo review can be part of a remote-care workflow, but the phrase does not establish that a particular service qualifies for a billing code or includes continuous surveillance. State exactly what the patient will send, how often it is requested, and what kind of review has been arranged.

02 · Make the record usable

Keep observations traceable.

Identify the wound, date, source, and measurement method. Preserve original images and make corrected entries distinguishable from the initial observation. Decide what happens when a photograph is missing, a different wound is photographed, or a new device produces an unfamiliar result. AI output should be labeled as such and accompanied by the clinician’s review where it informs care. A report that mixes patient statements, bedside findings, and software-generated suggestions without identifying them can obscure what is actually known.

03 · Define the response

An inbox needs an owner.

Agree on who reviews submissions, how absences are covered, and how concerning information reaches the treating clinician. Tell patients the expected review process and the separate route for urgent concerns. HHS advises establishing a telehealth emergency plan, including the patient’s location and a backup contact method. HHS: creating a telehealth emergency plan. CWS’s developing approach to remote wound information is physician-led; this website does not offer automated continuous monitoring. Technology should make the next clinical decision easier to trace, including a decision to examine the patient in person.
