From Telemedicine to Surgery: Building a Connected Wound-Care Pathway

A connected wound-care pathway gives each encounter a purpose. Telemedicine can open the conversation. In-person evaluation can answer questions that need direct examination. Surgery is considered when appropriate, within a plan that also accounts for ongoing care.

01 · Start with the question

Use the first encounter to orient the plan.

The referral should explain what is happening and why another level of review is being requested. The wound specialist can then identify which records and observations are available, what remains uncertain, and whether a virtual visit is suitable. A useful first encounter does not need to promise a particular treatment. It needs to establish the clinical question and who will help answer it.

02 · Evaluate before intervention

Keep surgery conditional.

If the patient needs in-person evaluation, the next visit should address the question that could not be resolved remotely. The treating clinician determines whether further diagnostic work, continued conservative care, or surgical treatment is appropriate. A telemedicine appointment is not surgical clearance, and a photograph does not by itself establish that surgery is needed. CWS’s escalation pathway is built around evaluation and clinical judgment rather than an automatic sequence that sends every wound to an operating room.

03 · Connect the next visit

Plan for the care that follows.

When a procedure is appropriate, the surrounding handoffs matter: sharing records, clarifying instructions, coordinating transport needs, and identifying who manages follow-up.
