Virtual Wound Care for Home Health Agencies
Home health teams see a wound in the context of everyday life. A virtual consultation can bring that context into a conversation with a wound specialist, while keeping clear responsibility for the patient’s ongoing care.
Describe care between visits.
A useful referral explains the wound concern and the practical situation: who helps the patient, what records are available, and what makes follow-up difficult. The patient’s own account belongs alongside the visiting clinician’s observations. A specialist also needs to understand which clinician currently directs the wound plan. These details help frame the consultation without assuming that the patient or caregiver can perform clinical tasks independently.
Make participation manageable.
Arrange the appointment around the patient’s communication and mobility needs, and clarify whether a home health professional will attend. Test the connection and decide how the requested records will be shared. HHS describes telehealth as a way to connect people with providers for ongoing care, including specialist access. Agree on follow-up before disconnecting.
The consultation should leave the patient and agency clear about who is issuing treatment orders, who will review new information, and how the next appointment will happen. Ask what to do if a scheduled home visit changes or the video connection fails. A request for remote review does not mean someone is continuously watching the wound.
